You need at least adobe reader version 7 in order to use this document.
This form can be completed on-screen by typing content directly into the PDF document.
Use the Tab key to move through the highlighted fields.
Date fields can be completed either by using the drop-down arrow on the right hand side of the field, clicking the current year then using the drop-down arrow to select the year Or by entering a date manually, using the format dd/mm/yyyy.
Fields requiring numerals, eg iwi codes, will be transposed into zeros if you enter letters.
All sections must be completed.
After you have finished
Remember to enter your name in the signature field and date your completed form.
You may wish keep a copy of this form for your personal records, so once completed, save a copy to your computer.
You can submit the form electronically by pressing the button [Submit by Email].
If you encounter problems using the [Submit by Email] button, save the form to your computer and then manually email the file as an attachment to the address mentioned at the top of the application form.
